Does Zepbound Need to Be Refrigerated? How Long Tirzepatide Lasts Out of the Fridge
Yes, Zepbound is stored in the refrigerator. The more useful question, and the one people are usually asking, is how long it survives outside one: the pen left on the counter overnight, the vial in a hotel room, the week away from home. The label answers that with a number, and the number is more generous than most people assume. This page gives the label figures for Zepbound, Mounjaro, Wegovy and Ozempic, then covers compounded and research vials, which have no label and follow different rules.
The short answer
Zepbound pens and vials live in the fridge at 36 to 46 F (2 to 8 C). Once out, the label allows up to a total of 21 days at room temperature not above 86 F (30 C) for the single-dose pen or vial, and 30 days for the KwikPen and multi-dose vial. Never frozen, out of direct light, back in the fridge is fine within those days. Compounded tirzepatide has no label allowance: keep it cold and follow the pharmacy's beyond-use date. A reconstituted research vial should be treated as a fridge-only item.

What the 21 days actually means
The allowance is cumulative time out of the fridge, not a fresh 21 days each time the pen comes out. A pen that spent a week on a trip and goes back in the fridge has 14 days of room temperature left in it. It is a ceiling, not a target: the label says to discard the pen or vial after the 21 days.
The 86 F limit matters more than the day count in practice. A parked car in summer passes 86 F in minutes. A checked suitcase in a hold can go either way. A kitchen counter in a normal home does not come close. Past 86 F the label no longer covers the pen, and a pharmacist or Lilly is the right call.
Does it need to be at room temperature when you inject it?
No. The patient instructions ask the question and answer it: it is not necessary to warm the pen to room temperature. Whether you let it sit first is a comfort choice, and any time out counts toward the 21 days in theory. Compounded tirzepatide is a different case
A compounded vial from a 503A or 503B pharmacy has no manufacturer stability study behind it. Its storage rules are the pharmacy's beyond-use date and its own label, which usually says refrigerate and gives a short window at room temperature, if any. Some pharmacies say up to a few days; some say none. Treat the pharmacy's figure as the answer and the branded 21 days as irrelevant to your vial. Research-grade tirzepatide bought as lyophilised powder follows the general rules in the peptide storage guide: powder is tolerant and lasts months cold and dry; once reconstituted with bacteriostatic water it is a two to four week item in the fridge, and room temperature is for the minutes around an injection, not for storage.
Travelling with it
For most trips the label does the work: 21 days at room temperature covers a holiday, and a hotel fridge covers the rest. Carry it in your hand luggage, never the hold, and keep it out of the sun and the car. Where a cooler earns its place is heat and length: a summer road trip, a destination above 86 F, a hotel with no fridge, or vials rather than pens. Signs a pen or vial should be discarded
- It has been frozen, even once.
- It has spent time above 86 F, or more than the label's days in total out of the fridge (21, or 30 for the KwikPen and multi-dose vial).
- The liquid is cloudy or has particles in it. The labels describe semaglutide as clear and colorless and tirzepatide as colorless to slightly yellow, so a pale yellow tint on tirzepatide is normal.
- It is past the expiry date on the carton.
